This report was compiled through several files, audio/video logs, and firsthand accounts to form a comprehensive record of events.

Patient 0 - Initial Contact


On the morning of 03/26/2014, Emanuel Nunez, a courier operating out of Base Omega, failed to report to a scheduled meeting to receive his next assignment. That afternoon his supervisor sent someone to check on Nunez in his barrack. He was discovered in an unlikely state. A large portion of his skull had completely dissolved, with only his lower jaw remaining. The skin around Nunez's neck and upper chest had begun the process of converting to gingival tissue. Despite his condition, his vitals remained stable, though he was unresponsive to external stimuli. Emanuel Nunez was promptly taken to the Base Omega infirmary for observation.

"…After noting the most obvious afflictions, my attention was drawn to his remaining jaw. The teeth were nearly all missing, and the sockets showed signs of rot, comparable to extreme tooth decay. We have been seeing a lot of cavities amongst Base Omega personnel over the last month; Nunez himself was scheduled for a tooth extraction after his next mission… As coincidental as it seems, my immediate concern was that some sort of backborn tooth infection was spreading through the base—if only it was that simple…"

Dr. Adebayo continued to observe Nunez throughout the afternoon and evening. During this time the spread of gingival tissue rapidly transfigured Nunez. The doctor used this opportunity to analyze the saliva that was being secreted, leading to the first documentation of the Entity 116 bacteria. Adebayo immediately requested that all other couriers with cavities report to the infirmary—of the eight, only seven responded to the summons.

Maintenance Report

Drinking fountain in the Couriers' Barrack; Base Omega.

The drinking fountain in the couriers' barracks was immediately disassembled and inspected. The maintenance worker was given a hazmat suit, and Dr. Adebayo oversaw the inspection directly. Almost immediately, the maintenance specialist recognized that the filter used was not the standard filter typically issued by the M.E.G.

The oddity was identified as an in-line additive canister, filled with an unidentified, cloudy liquid. Tests conducted by Dr. Adebayo confirmed that the canister was filled with a calcium-based solution that had an alarming concentration of the bacteria.

Following this discovery, the security team placed Base Omega under a partial lockdown. All fountains within Base Omega were ordered to be inspected, though no further sabotage was discovered.

Autopsy Summary


Name: Timothy Fredrick

Time|Date: 01:20|3/27/2014


The deceased suffered from several strikes to the occipital region, the final of which left the murder weapon (a small stainless steel surgical tray) embedded in the skull. Hairline fractures and bruising on the forehead and temple are consistent with a forward fall after being struck from behind. Blood on the edge of the table lends validity to this theory.

Bruising on Timothy's stomach is indicative of knee impressions from the attacker kneeling on him. Using his neck as leverage, it would appear that significant force was used to rip the deceased's lower jaw from his skull—further confirmed by tearing of the skin, ligaments, and muscles of the face and neck.

Likely cause of death: catastrophic brain injury, hemorrhaging.

Summary: Midnight Massacre


On the morning of the 28th, a total of eight residents within Base Omega were discovered brutally murdered in their barracks. Each was bludgeoned to death in their sleep before their teeth and lower jaws were removed in a similar fashion to Timothy Fredrick.

Doug Westfield, a security specialist, managed to fight off his attacker, claiming a "skinless man with no head" assaulted him during the night. Westfield described the assailant wearing M.E.G.-issued clothing. He managed to strike the humanoid entity several times with the lamp he kept on his end table. After he gained the upper hand, the attacker fled through his open door.

After-Action Report


Filing: Base Omega Security Team

Subject: Elimination of unregistered entity.

Time|Date: 11:30 MST|03/28/2014


Security specialist Danielle Melrose traced a trail of saliva leading from one of the victim's rooms into an adjacent unoccupied barrack. The trail led to a louvered closet door. Melrose could see the entity through the door; not wanting to risk a direct encounter, she engaged without opening it.

She was armed with a baton rigged to fire a single 20-gauge firesalt cartridge. Placing the baton at eye level, she fired a round into the closet, hitting the entity in the upper chest and throat. As the entity scrambled from the closet, Melrose exited the room and locked the door.

The entity died after a few minutes of struggling, at which time its body was recovered.


Operation Successful


The entity was confirmed to be Miguel Barrera, the previously missing courier. He had been fully transfigured, with nearly his entire body covered in gingival tissue. Much like Mr. Nunez, he was missing a large portion of his skull. A total of sixteen teeth and two and half jaw bones were found in his digestive tract. This was not nearly enough to account for all of the deaths from the previous night. Nunez is likely still out there, waiting for his chance to strike… Of even more concern, though, are the seven men currently laying in the infirmary—all likely to transform in a similar way.

After-Action Report

Subject: Acquisition of Emanuel Nunez

Upon arriving, Team Karl, in conjunction with local security, performed a thorough sweep of Base Omega. While clearing a barrack, Base Omega security officer Maria Brokaw noticed a figure hiding beneath one of the beds. Brokaw radioed in their location before attempting to apprehend Nunez themself, against the direct order of Team Karl's captain.

Team Karl converged on the location and found Brokaw incapacitated with the transfigured Nunez midway through removing her teeth. The team managed to overwhelm and capture Nunez using a combination of stun batons and snare poles.

A local medic, Dr. Adebayo, stabilized Brokaw enough that she could be transferred with the other subjects.

Conclusion


In total, this attack by the Church of the Veil took the lives of sixteen members of Base Omega. This was the first of many attacks orchestrated by the group, many of which involved the biological weapon that would later be designated Entity 116. Despite the tragedy surrounding this attack, the information gained through it was instrumental in mitigating future disasters.

Though the conflict had been smoldering for over a year by this point, the attack on Base Omega spurred an immediate response from the newly formed Oversight to take the Veiled threat seriously. New teams, outposts, and alliances were formed in haste to prepare for an extended conflict with the group.
